Notice Regarding Request for Affordable Care Act (ACA) 1095-C Forms

Due to changes in the tax guidelines, ACA 1095-C forms are no longer required when filing your Federal Income Taxes. Additionally, employers are no longer required to automatically mail copies of these forms.

Current employees can access their 1095-C in Employee Online. If you are a former Clawson Public Schools employee and would like a copy of your ACA 1095-C form, you must request it by emailing Edna John.
